PRIVACY AND YOUR PERSONAL AND HEALTH INFORMATION
Personal and health information is information or an opinion about an individual who can be identified. We collect your personal information so that we can:
- Understand your requirements and identify the relevant program, support and assistance that is right for you;
- Plan and improve our services;
- Ensure adequate risk reduction for all concerned;
- Manage complaints, disputes or legal proceedings;
- Prepare funding submissions and reports;
- Undertake research and compile or analyse statistics;
- Promote our services;
- Improvement awareness and understanding of the criminal justice system and advocate for new services or policy development.
This information may include referrals and assessments, case-notes mental health, medical history, offending history, court reports, supervision notes and case reviews.
WHAT YOU CAN EXPECT FROM US
You will be informed about the personal and health information we need to collect about you, why we collect it and how it will be used and handled;
Your information will be kept accurate, up to date as far as possible, secure and confidential;
Limitations to confidentiality will be explained to you;
We will only release identifiable information about you if you agree or if required by law, such as in a medical emergency, or if there is a serious threat to your health and well-being or that of someone else;
If we use your information for reporting and planning, it will not be identifiable.
YOUR RIGHTS AND RESPONSIBILITIES
You can access the information that we collect about you, except where this is prohibited by law. You can also correct your information if you believe it is inaccurate, not up to date, misleading or not complete. We will respond to you within 30 days of receiving your request.
To assist us keep your information up to date, we ask you to notify us promptly of any change of address, contact details or circumstances.
